Please complete this form accurately and choose your weight class wisely.  Be sure to review Medicals and Weight Classes before you begin. 

You will be required to upload a photo.  If you get a bout, we will need this picture for your fight profile on PPV and for advertising, so please upload a clean, high-resolution picture against a solid-color background.  Please have this ready to complete your submission.

Amateur fighters, your chosen weight class must be within 15lbs of your walkaround weight.  

We will begin notifying fighters about potential bouts beginning 10/1/25, if not sooner.

  • Confirm your availability and clear it with your coach.
  • Review all Rules, Required Gear, Medicals & Licensing.


Medical Requirements (Due 3 Weeks Before the Event):

  • Blood Work (HIV Panel, Hep B Surface Antigen, Hep C Antibody)

  • Dilated Eye Exam (must be on NCBCS form – (Download Here)
    Can be done at any vision center (Walmart, Target, etc.)
